Atrium Health's Carlyle Place in Macon, GA is seeking an LPN to provide nursing care across Skilled Nursing, Assisted Living, and Memory Care. The LPN works under RN supervision to support comfort, wellness, and quality outcomes in a premier not-for-profit life plan community.

• Graduate of an accredited School of Practical Nursing.
• Current Georgia LPN license (or compact eligible).
• CPR certification or ability to obtain within 60 days of hire.
• BLS certification required per policy.
• Skilled nursing, long-term care, assisted living, or memory care experience preferred.

Responsibilities

• Provide compassionate nursing care to residents in Skilled Nursing, Assisted Living, and Memory Care settings.
• Monitor resident conditions and report significant changes to the healthcare team.
• Assist with assessments, care planning, treatments, and procedures.
• Administer medications and treatments within scope of practice.
• Educate residents and families regarding care plans and treatments.
• Document assessments, interventions, and resident responses accurately and timely.
• Collaborate with interdisciplinary team members to support positive resident outcomes.

Position Summary

The Licensed Practical Nurse (LPN) provides nursing care and assists in the planning and implementation of resident care focused on comfort, wellness, and quality outcomes. Working under the supervision of a Registered Nurse or licensed practitioner, the LPN participates in assessments, delivers treatments, monitors resident conditions, and contributes to individualized care plans.
