Job summary

West State Street is seeking a licensed practical nurse (LPN) for our Assisted Living community. You will work 11 PM to 7 AM under an RN, delivering nursing care that supports residents' vitality and independence.

You will document data, contribute to care plans, administer medications, and ensure smooth communication with RNs and physicians, while supervising unlicensed staff and using PCC for accurate records.

Qualifications

• Must be a graduate of an approved school for practical or vocational nursing.
• Current LPN license by the State Board of Nursing in the state of employment.
• Clear English reading/writing/speaking for patient safety.

Responsibilities

• Collect, report, and document patient data; observe and report changes to the RN.
• Contribute to individualized care plans with goals and interventions.
• Administer medications and treatments as ordered by physicians.
• Communicate pertinent data to the RN/physician.
• Supervise care by unlicensed staff per policies and procedures.
• Utilize PCC for documentation and maintain accurate drug counts.
