Job summary

Heritage Manor Bossier is seeking a dedicated LPN to join our night shift team (11pm–7am). You will assist in implementing resident care plans, administer medications, monitor patients, and support RNs while ensuring accuracy in medical charts.

Active LPN license in the applicable state is required. Experience in geriatric care is preferred, and strong communication skills help coordinate care with physicians, nurses, and families.

Responsibilities

• Patient Care: Assist in the implementation of resident care plans and provide basic medical care, such as dressing wounds and taking vital signs.
• Medication Administration: Administer medications and monitor for side effects.
• Support RN: Assist RNs in carrying out care plans and follow their directives.
• Resident Observation: Monitor residents conditions and report changes to the RN or physician.
• Documentation: Record patient information and updates accurately in medical charts.
• Supportive Tasks: Assist with resident mobility and personal care activities as needed.
