Job summary

Oakwood Community Living Center is seeking a Licensed Practical Nurse (LPN) to join our team for a full-time night shift from 6pm to 6am. Responsibilities include medication administration, treatments, ADL care, and ongoing rounds to ensure high-quality patient care.

You will document medical records accurately and communicate with residents, families, physicians, and supervisors. Requirements include an active LPN license in good standing and BLS certification.

Qualifications

• Licensed Practical Nurse (LPN) license in good standing is required.
• BLS certification is required.

Responsibilities

• Evaluation and delivery of care including medication administration, treatments, and ADL care as ordered.
• Conduct regular rounds to monitor residents and ensure quality care.
• Communicate with residents, family members, physicians and supervisor regarding resident status.
• Maintain up-to-date medical records during tour of duty on assigned unit.
• Other tasks as required.
