Job summary

Onyx Care of Farmerville is seeking a Licensed Practical Nurse to join our dedicated team. This 6p-6a shift (Mon-Thurs) provides around-the-clock skilled nursing care in a post-acute rehabilitation setting, focusing on individualized patient goals and collaborative care.

You will administer medications, monitor vitals, and document care while supporting a clean, safe clinical environment. Louisiana LPN licensure and strong communication skills are required, with prior post-acute experience

Qualifications

• Current LPN/LVN licensure in Louisiana.
• Associates or Bachelor's in Nursing or related field preferred.
• Prior experience in post-acute rehabilitation or long-term care setting preferred.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
• Strong attention to detail and ability to maintain accurate records.

Responsibilities

• Provide high-quality patient care in a fast-paced post-acute rehabilitation setting.
• Collaborate with multidisciplinary healthcare teams to develop and implement individualized care plans.
• Administer medications, treatments, and other medical interventions as prescribed by physicians.
• Monitor patient vital signs, symptoms, and conditions, and report changes to the healthcare team.
• Assist with patient transfers, ambulations, and other mobility exercises.
• Maintain accurate and up-to-date patient records and documentation.
• Support the care team in maintaining a clean, safe, and organized clinical environment.
