Job summary

Onyx Care of Farmerville in Louisiana is seeking a Licensed Practical Nurse to cover 6a-6p shifts Monday through Thursday. You will deliver patient-centered care in a post-acute rehabilitation setting, utilizing advanced technology to support daily nursing duties and promote recovery.

You will administer medications and therapies, monitor patient progress, and collaborate with the interdisciplinary team to ensure smooth transitions.

Qualifications

• Louisiana LPN/LVN license is required.
• High school diploma or equivalent is required; ADN or higher is preferred.
• Minimum 0 years of experience in a post-acute rehabilitation setting or similar healthcare environment.

Responsibilities

• Provide high-quality, patient-centered care to our residents, promoting their physical, emotional, and psychological well-being
• Administer medications, treatments, and therapies as prescribed by our medical team
• Monitor and report patient progress, identifying areas for improvement and developing personalized care plans
• Collaborate with our interdisciplinary team to ensure seamless care transitions and effective communication
• Maintain accurate and detailed records of patient care, medication administration, and treatment outcomes
• Perform routine tasks, such as vital sign monitoring, wound care, and assistance with activities of daily living (ADLs)
