Onyx Care of Farmerville is seeking a Licensed Practical Nurse for the 6p-6a shift (Mon-Thurs) to join a dedicated post-acute rehabilitation team. You will deliver around-the-clock skilled nursing care, collaborating with a multidisciplinary team to develop and implement individualized care plans.

The ideal candidate holds a Louisiana LPN/LVN license, with a background in post-acute rehab or long-term care, and excellent communication skills for fast-paced settings.

Qualifications

• Current Louisiana LPN/LVN licensure is required.
• High school diploma or equivalent; college degree preferred.
• Experience in post-acute rehab or long-term care is a plus.

Responsabilités

• Provide high-quality patient care in a fast-paced post-acute rehab setting.
• Collaborate with multidisciplinary teams to implement individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor vital signs and report changes to the care team.
• Assist with transfers, ambulations, and mobility exercises.
