As a key member of our nursing team, you will play a vital role in delivering around-the-clock skilled nursing care to our patients. Responsibilities:
Provide high-quality patient care in a fast-paced post-acute rehabilitation setting
Collaborate with multidisciplinary healthcare teams to develop and implement individualized care plans
Administer medications, treatments, and other medical interventions as prescribed by physicians
Monitor patient vital signs, symptoms, and conditions, and report changes to the healthcare team
Assist with patient transfers, ambulations, and other mobility exercises
Maintain accurate and up-to-date patient records and documentation
Support the care team in maintaining a clean, safe, and organized clinical environment
Requirements:
Current LPN/LVN licensure in the state of Louisiana
High school diploma or equivalent required; Associates or Bachelor's degree in Nursing or related field preferred
Prior experience in post-acute rehabilitation or long-term care setting preferred
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
Strong attention to detail and ability to maintain accurate records
