Job summary

Willoughby SNF Healthcare, LLC is seeking an LPN/LVN supervisor to provide direct nursing care to residents and oversee day-to-day nursing operations in a long-term care facility in Ohio. Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
• Must possess a current CPR certification.
• Knowledge and experience with PCC preferred.

Responsibilities

• Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
• Ensure all nursing personnel comply with written policies and procedures established by the facility.
• Review department policies, procedure manuals, and job descriptions; recommend revisions.
• Plan shifts’ services, programs, and activities with nursing staff and support personnel.
• Maintain Nursing Service Procedures Manual and ensure compliance with it.
• Provide written and oral reports/recommendations about shift activities.
• Coordinate nursing services with other resident services for the resident’s total care.
• Care planning for resident discharge; assist in discharge plan updates.
