Job summary

Mercy Rehabilitation Hospital - Springfield is seeking a Licensed Practical Nurse (LPN) for FT Nights. You will deliver direct and indirect patient care using the nursing process, and collaborate with a multidisciplinary team to support patients' recovery.

The role emphasizes discharge planning, communication with physicians, families, and guardians, and providing compassionate, evidence-based care in an inpatient rehabilitation setting. LPN licensure and BLS required; ACLS preferred.

Qualifications

• Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Licensed Practical Nurse.

Responsibilities

• Deliver direct and indirect patient care using the nursing process-Assessment, Planning, Intervention, and Evaluation.
• Collaborate closely with patients, families, and our interdisciplinary care team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues.
• Play a key role in the discharge planning process to support safe, successful transitions.
