Mercy Rehabilitation Hospital - Springfield is hiring a Licensed Practical Nurse (LPN) for full-time day shifts. You will deliver patient care using the nursing process, collaborate with an interdisciplinary team, and monitor patient conditions to communicate changes to the care team.

Qualifications

• Graduate from an accredited nursing program and current LPN licensure.
• CPR/BCLS certification; ACLS preferred.
• 6 months Medical/Surgical experience in an acute care setting preferred.

Responsibilities

• Deliver direct and indirect patient care using the nursing process: assessment, planning, intervention, and evaluation.
• Collaborate with patients, families, and the interdisciplinary care team to create personalized care plans.
• Monitor and communicate changes in patient conditions to physicians, supervisors, and colleagues.
• Play a key role in discharge planning to support safe, successful transitions.
