Job summary

Union Post Acute is seeking a South Carolina licensed practical nurse (LPN) to join our long-term care and rehabilitation community in Union, SC. This role offers 12-hour night shifts (7:00 PM–7:00 AM) with shift differentials and opportunities for advancement.

You will provide direct nursing care, administer medications, monitor residents, and document care while collaborating with CNAs, nurses, physicians, and the care team to ensure quality resident care.

Qualifications

• Active and unencumbered South Carolina LPN license.
• Current CPR certification.
• Graduate of an accredited Practical Nursing program.
• Long-term care experience preferred, but not required.
• Strong clinical and communication skills.
• Compassion, reliability and commitment to resident care.

Responsibilities

• Provide direct nursing care to residents.
• Administer medications and treatments as ordered by physicians.
• Monitor resident conditions and report changes.
• Collaborate with CNAs, nurses, physicians and the interdisciplinary team.
• Maintain accurate and timely documentation.
• Create a safe, caring environment and build relationships with residents and families.
