Qualifications

• Licensure: Valid LPN/LVN license in Nebraska or Multistate license.
• Education: Graduation from an accredited LPN/LVN program.
• Experience: Preferred in a skilled nursing facility or similar setting.
• Certifications: CPR/BLS required; other certs per state regulations.
• Communication: Strong verbal and written skills; teamwork.
• Physical: Ability to lift/move patients and stand long periods.

Responsibilities

• Provide direct nursing care to residents, including medications, wound care, and treatments as prescribed.
• Monitor vital signs, document progress, and report changes to Charge Nurse or DON.
• Assist in development and implementation of individualized care plans.
• Maintain accurate documentation and communicate with residents, families, and team.
• Adhere to facility policies, infection control, and safety protocols.
• Provide emotional support and education to residents and families.
• Collaborate with RNs, CNAs, physicians, and therapists for coordinated care.
