Job summary

Emerald Nursing & Rehab Mercy in Omaha, Nebraska seeks an LPN/LVN to provide compassionate, resident-centered care on day and night shifts. You will assess needs, deliver direct nursing care, administer medications, and collaborate with RNs, CNAs, physicians, and therapists to achieve timely, quality outcomes.

The role emphasizes teamwork, effective communication, infection control, and adherence to care plans, with a focus on resident well-being and education for families.

Qualifications

• Valid LPN/LVN license in Nebraska.
• Graduation from an accredited LPN/LVN program.
• Current CPR/BLS certification required.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
• Monitor residents' vital signs, document progress, and report significant changes to the Charge Nurse or Director of Nursing.
• Assist in development and implementation of individualized care plans tailored to each resident's needs.
• Maintain clear documentation and communicate with residents, families, and the healthcare team.
• Adhere to facility policies, infection control, and safety protocols.
• Provide emotional support and education to residents and families about care plans and health conditions.
• Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated care.
