Job summary

Quality Center for Rehabilitation and Healing seeks a compassionate Licensed Practical Nurse (LPN) for the night shift 6:00 p.m.–6:00 a.m. You will deliver direct nursing care in a rehabilitation and long-term care setting, following each resident's care plan with accuracy and empathy.

The successful candidate holds an unrestricted LPN license in the state, graduated from an approved LPN program, and is willing to work nights.

Qualifications

• Unrestricted LPN license in the state of employment.
• Graduate of an accredited practical nursing program.
• Current CPR certification or willingness to obtain it, as required.
• Previous skilled nursing, rehabilitation, or long-term care experience preferred.

Responsibilities

• Provide direct nursing care in accordance with residents' care plans.
• Administer medications and treatments as ordered by physicians.
• Monitor residents' conditions and promptly report changes to the clinical team.
• Accurately document medications, treatments, observations, and resident responses.
• Assist with admissions, transfers, and discharges.
• Communicate with residents, families, physicians, and the interdisciplinary team.
• Supervise and provide guidance to Certified Nursing Assistants as assigned.
• Follow infection-control, safety, and emergency procedures.
• Maintain residents' dignity, privacy, comfort, and confidentiality.
• Participate in care planning, shift reports, and quality-improvement initiatives.
• Perform other nursing duties as assigned.
