Qualifications

• Graduate of accredited school of nursing.
• Current LPN license or equivalent state licensure.
• BLS certification preferred or required within 3 months of hire.

Responsibilities

• Promotes positive relations with patients, visitors, families and staff.
• Greet patients and introduce self by name.
• Assist RN with initial patient assessment and ongoing observation.
• Complete initial assessments and documentation per nursing standards.
• Communicate changes in patient status to team leaders.
• Administer medications safely and accurately per policy.
• Document care and patient responses according to procedure.
• Participate in discharge planning and patient education.
