Genacross Lutheran Services is seeking an LPN to join Genacross - Wolf Creek | Skilled Nursing on a Full Time Night Shift. Compensation ranges from $26 to $28.80 per hour and varies with experience and shift differentials.

The role involves administering medications, monitoring residents, supervising STNAs, coordinating care with the clinical team, and maintaining accurate records while upholding safety and infection prevention standards.

Qualifications

• Current State of Ohio LPN license.
• One or more years of experience in a long-term care setting preferred.
• Basic computer skills and ability to learn new software applications.

Responsibilities

• Administer medications, treatments, and nursing services according to physician orders and care plans.
• Monitor resident health, perform assessments, and report changes in condition to the appropriate clinical team.
• Supervise and provide guidance to STNAs to ensure safe, effective, and resident-centered care.
• Coordinate care with physicians, pharmacists, residents, families, and interdisciplinary team members.
• Complete resident admissions, documentation, charting, and other required clinical records accurately and timely.
• Promote resident safety and well-being by following infection prevention, emergency response, and regulatory compliance procedures.
