Job summary

Oak Hill in Watertown, CT is seeking a Licensed Practical Nurse to support community programs, with 8–16 hours per week and a night shift schedule. You will provide clinical care, monitor health, document outcomes, and work with an interdisciplinary team to support individuals with disabilities.

The role emphasizes safety, advocacy, and independence, including medication administration, health monitoring, and coordination of appointments.

Qualifications

• Connecticut LPN license in good standing (CT)
• Graduate of an accredited Practical Nursing program
• Valid driver’s license for transporting individuals
• CPR and First Aid certification, or ability to obtain on hire
• Strong clinical assessment, communication, and documentation skills
• Ability to pass required background checks and health screenings
• Ability to maintain required certifications and regulatory requirements

Responsibilities

• Provide clinical and nursing care, monitor health, document observations, and administer prescribed interventions
• Coordinate medical appointments and communicate with physicians and teams
• Promote health, safety, and wellness through assessments and preventive care
• Support daily living activities, independence, and meals during programs
• Maintain nursing notes, records, and regulatory compliance
• Apply First Aid, CPR, lifting techniques, and safe transport
• Collaborate with team members and mentor new staff
