Oak Hill is seeking a Licensed Practical Nurse (LPN) for community programs in Watertown, CT. This role involves providing clinical nursing care, health monitoring, documentation, and direct support on weekends with 8-16 hours per week. A sign-on bonus of up to $4000 is offered.

Qualifications

• Current Connecticut LPN license in good standing.
• Graduate of an accredited Practical Nursing program.
• Valid driver's license to transport individuals in agency vehicles.
• Current CPR and First Aid certification (or ability to obtain).
• Ability to pass background checks, fingerprinting, and health screenings.
• Ability to maintain required certifications and regulatory requirements.

Responsibilities

• Provide clinical nursing care and monitor health needs.
• Follow physician orders and nursing directives for care and treatment.
• Administer medications, injections, and prescribed interventions.
• Document observations, vital signs, and nursing notes accurately.
• Support G-Tube care and other physician-directed health interventions.
• Coordinate medical appointments and liaise with healthcare providers.
