Job summary

Stonecreek in Paducah, KY is hiring for a Night shift LPN/RN to provide compassionate, patient-centered care in a 90-bed community. You will collaborate with the healthcare team to assess, plan, implement, and evaluate care plans, administer medications, and monitor patient status.

The role emphasizes bedside care, timely documentation, and participation in discharge planning. Qualified candidates have an active LPN/RN license and LTC experience; MatrixCare knowledge is a plus.

Qualifications

• Current LPN or RN license in the state.
• At least 1 year of LPN/RN experience in a Long‑term Care setting preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Consult with healthcare team to assess, plan, implement, and evaluate patient care plans.
• Prepare and administer medications and record responses.
• Record patients’ medical information and vital signs.
• Monitor equipment and interpret electronic displays.
• Initiate corrective action for adverse symptoms.
• Provide bedside care for a variety of medical patients.
• Assist with insertions and changes of dressings, catheters, and IVs.
• Assist physicians during examinations and treatments as needed.
• Participate in discharge planning and ensure accurate timely documentation.

LPN/RN Core Responsibilities:

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans
• Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.
• Records patients medical information and vital signs.
• Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays.
• Initiates corrective action whenever the patient displays adverse symptomatology
• Provides bedside care for a wide variety of medical patients.
• Changes dressings, inserts catheters, and starts IVs
• Prepares equipment and aids the physician during the examination and treatment of the patient
• Responds to life‑saving situations based upon nursing standards and protocol
• Participates in discharge planning.
• Utilizes time management skills.
• Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format, and on the appropriate forms.
• Performs other position‑related duties as assigned, depending on assignment setting

LPN/RN Requirements:

• Current LPN/RN license in State.
• At least one year of LPN or RN experience in a Long‑term Care Setting is preferred.
• Knowledge of MatrixCare preferred.
