Job summary

Stonecreek Health and Rehabilitation in Paducah, KY, is seeking a dedicated CNA to provide compassionate resident care on day or night shifts. The role includes assisting with activities of daily living, monitoring vitals, and reporting changes to the nursing staff.

Qualifications

• Provide direct resident care including bathing, feeding, dressing, and ambulating.
• Record and monitor vital signs and move residents safely.
• Recognize changes and report to nurse manager promptly.
• Communicate clearly with residents, families, and staff.
• Assist nurses with duties as directed.

Responsibilities

• Attend to the individual needs of residents, including bathing, feeding, dressing, and ambulating.
• Responsible for taking and recording patient vital signs, lifting, moving, and transporting residents, using proper body mechanics.
• Recognizing and reporting changes in resident conditions to a nurse manager. Asking appropriate questions to clarify situations.
• Communicating and interacting effectively and tactfully with the residents, visitors, families, peers, and supervisors.
• Reporting abnormal changes or patterns to nursing staff.
• Demonstrating the ability to assist the nurse with various duties as directed.

CNA's are responsible for the following:

• At least one year of experience as a CNA in a long-term care setting is highly preferred.
