Job summary

Sycamore Heights Health and Rehabilitation in Kentucky is seeking an LPN to provide bedside care, monitor vital signs, administer medications, and relay information between patients, RNs and physicians.

You will perform wound care, catheter care, urine collection, charting, and ensure confidentiality while delivering compassionate, patient-centered care in a long-term care setting.

Qualifications

• Current LPN license in Kentucky.
• At least 1 year of LPN experience in a long-term care setting preferred.
• Strong clinical skills and patient communication.

Responsibilities

• Provide bedside care to patients, including personal hygiene maintenance.
• Record vital signs (blood pressure, pulse, respiratory rate).
• Collect patient health information.
• Relay information between patients, registered nurses, and physicians.
• Give oral medications and injections to patients.
• Test a patient’s blood sugar levels.
• Urinary catheter insertion and care.
• Wound care and dressing changes.
• Assist patients with personal hygiene.
• Monitor for changes in patients.
• Proper charting, documentation, and maintenance of patients’ medical records.
• Ask questions when in doubt for clarification by the Registered Nurse.
• Act as a role model for ancillary personnel.
• Maintain the confidentiality of the patient’s clinical records.
• Exceptional staff training and customer service skills.
• Compassionate and friendly demeanor.
