Parkland Community Health Plan in Dallas, TX is seeking a Pharmacist to provide hospital pharmacy services on the night shift. The role involves order review, drug safety evaluation, formulary management, and collaboration with physicians, nurses, and staff.

Qualifications

• Must have a bachelor's degree in pharmacy or Pharm. D degree from an accredited school of pharmacy.
• Must be a Registered Pharmacist in the State of Texas.
• Must possess good oral and written communication skills.
• Must have basic computer skills.
• Must possess knowledge of drugs, procedures, equipment, common disease states and patient care monitoring within the assigned care areas.
• Required appropriate pharmacy specific mathematical skills.
• Must possess good organizational skills.

Responsabilités

• Demonstrates effectiveness and efficiency in routine order preparation, review & verification by authorized prescribers for therapeutic appropriateness, duplications, side effects, effectiveness, lab monitoring, etc.
• Facilitates and ensures appropriate formulary use. Resolves routine prescribing, formulary or therapeutic issues.
• Performs computer entry/verification & preparation of medication orders and processing including specialty orders. Ensures accurate and timely entry of medication orders and patient charges.
• Promotes cost-effective therapies and implements cost containment strategies.
• Documents interventions for routine resolved prescribing, formulary & therapeutic issues and discusses adverse drug events with the care team.
• Provides training and education to pharmacy students, residents, technicians, and new staff as needed.
