Job summary

The Elizabeth Hospice is seeking a Triage RN to perform skilled nursing visits, assessments, and care across ages to maximize comfort in accordance with TEH protocols. The role involves travel within San Diego County and South Riverside, CA, and requires on-call shifts monthly.

Full-time nights, 4/10 hour shifts per week, with hours 9:30 p.m. to 8:00 a.m. California RN licensure, CPR, and TB test are required, with hospice experience preferred.

Qualifications

• Associates degree required; bachelor's preferred.
• Minimum 12 months of acute care experience; hospice experience preferred.
• Current CA RN license.
• Current CPR certification.
• TB test negative.
• Bi-lingual (Spanish) preferred.

Responsibilities

• Performs visits as directed by the clinical team manager.
• Responds to requests for patient visits promptly.
• Independently assesses patient/family care needs and provides care as ordered.
• Instructs patients, caregivers and family members.
• Supervises LVN and HA per plan of care and TEH policy.
• Maintains timely patient documentation in electronic charting.
• Collaborates with physicians and supervisors for order changes.
• Adheres to E-way standards in all duties.
