Job summary

Compassus, a leader in home-based hospice and palliative care, seeks a Hospice Registered Nurse (RN) for Evening/Night shifts. The role involves assessing patient needs, delivering skilled nursing care, and coordinating with the IDG to plan care for home-bound patients, including end-of-life support and symptom management.

Responsibilities include creating and updating the Plan of Care, on-call duties, and maintaining accurate clinical documentation.

Qualifications

• Associate or Bachelor's degree from an accredited School of Nursing.
• Preferred - 1 year experience practicing as a registered nurse.
• Experience as a Hospice CNA or Hospice LPN will be considered.

Responsibilities

• Completes assessment of patient and family to determine Hospice needs.
• Provides a complete physical assessment and history and provides professional nursing care by utilizing all elements of the nursing process.
• Writing and initiating Plan of Care in collaboration with the IDG.
• Regularly reevaluating patient/family/caregiver needs and revising the Plan of Care as necessary.
• Initiates the Plan of Care and makes necessary revisions as patient status and needs change.
• Develops a care plan in conjunction with the patient/family/caregiver and IDG which establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
• May administer medications and treatments as prescribed by the IDG approved Plan of Care.
• Participates in on-call duties as defined by the on-call procedures.
• Maintains productivity standards as set by organization guidelines.
• Documentation/Information Management: Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
• Supervises ancillary personnel and delegates responsibilities when required.
• Participates in orientation, education, and development of team members.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
