Job summary

Hospice of Acadiana Inc. in Lafayette, LA is seeking an experienced Inpatient Unit RN to join the IPU team. You will perform comprehensive assessments, develop care plans, and deliver skilled nursing care to maximize comfort for patients and families in line with policy and nursing standards.

You will administer prescribed medications, educate patients and caregivers, coordinate with the interdisciplinary team, document care thoroughly, and participate in quality improvement initiatives.

Qualifications

• Bachelor's degree in nursing required.
• Two years RN experience preferred.
• Hospice experience preferred.

Responsibilities

• Performs initial and ongoing patient and family assessments to determine hospice needs.
• Develops and revises care plans based on nursing diagnoses and patient needs.
• Administers medications and treatments as prescribed by physicians.
• Educates patients and families on care, medications, and self-care.
• Coordinates patient care as case manager when assigned; communicates with physicians and team.
• Participates in interdisciplinary team meetings and quality improvement activities.
