Job summary

Bristol Hospice Northwest-Crown Point, IN is seeking compassionate Registered Nurses to serve as our Warriors of the Night, providing after-hours hospice care in Merrillville, IN 46410. This full-time graveyard shift offers autonomy and a chance to be a lifeline for patients and families during evenings, nights and weekends.

Qualifications

• Must be a Registered Nurse with hospice knowledge.
• Must hold current licensure in good standing with the state.
• Flexibility to work after hours, nights, and weekends.
• Ability to travel throughout the Bristol Hospice service area.
• Proficiency with basic computer software and data entry.

Responsibilities

• Perform after-hours and weekend visits and admissions as needed.
• Direct phone calls to the DPCS, RN Clinical Supervisor/Manager or RN Case Manager and/or handle urgent needs per scope of practice.
• Dispatch field nurses and other staff to patient residences in a timely manner.
• Refill prescriptions, order supplies and DME, and assist in coordination for patients and families.
• Document calls, conversations and outcomes in the software system.
• Receive start-of-shift report and give end-of-shift report per location workflow.
• All other duties as assigned.
