Qualifications

• ADN and/or BSN from an accredited institution.
• Two years of nursing experience, or equivalent combination of education and experience.
• Current MD registered nursing license.
• Proficient in infection control practices and safe patient handling.
• Experience using EMR systems or willingness to learn.

Responsibilities

• Provide quality nursing care to patients with life-limiting illness and support to families.
• Collaborate with the interdisciplinary team to create and implement patient-centered care plans.
• Monitor patient progress and adjust care as needed, educating patients and families.
• Document accurately in patient charts and records; maintain timely records.
• Participate in quality improvement initiatives to enhance patient outcomes.
• Supervise and direct Patient Care Partners and unit staff as needed.
• Attend to patient and family needs at end-of-life; support bereavement resources.
