Job summary

Hospice Of Hope, Inc. seeks an experienced Registered Nurse for inpatient care at our Seaman, Ohio center.

This full-time night shift role provides skilled nursing to terminally ill patients, with emphasis on assessment, planning and coordination of care. Responsibilities include creating and updating care plans, educating patients and families, documenting medical information, and ensuring regulatory compliance while collaborating with physicians and care teams on a three-shift weekly schedule

Qualifications

• Graduate of an accredited School of Nursing with current multi-state licensure or eligibility to be licensed in Kentucky or Ohio.
• At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility is preferred.

Responsibilities

• Assess patient needs using clinical tools, data and information.
• Create, implement and update individualized plans of care.
• Educate patients and caregivers on benefits, services and resources.
• Document complex medical information to outline and improve results.
• Follow quality care processes in compliance with regulatory guidelines.
• Communicate effectively with patients, physicians and other stakeholders.
• Assess and respond to patient and family needs in a timely manner.
• Work with basic computer tools including Microsoft Office.

Education

Graduate of an accredited School of Nursing
