Job summary

OakMed is seeking a compassionate Home Health Aide to provide one-on-one care for patients in their homes in East Grand Rapids, MI, during nighttime hours. This PRN field role offers flexibility and the chance to impact patients’ safety, comfort, and independence.

You will collaborate with nursing and care coordination teams, follow individualized care plans, and maintain accurate documentation. A high school diploma, valid driver's license, and ability to pass a background check are required.

Qualifications

• High school diploma or equivalent required.
• Valid driver’s license required.
• Ability to pass a criminal background check and work legally in the United States.
• Ability to work independently during nighttime hours.

Responsibilities

• Provide personal care: bathing, dressing, grooming, toileting, and daily living activities.
• Assist patients with mobility, transfers, positioning, and safe patient-handling practices.
• Reposition patients to promote comfort and prevent skin breakdown.
• Provide companionship and emotional support while maintaining patient dignity and privacy.
• Prepare meals and assist with basic nutritional needs per care plan.
• Perform light housekeeping to maintain a safe, clean home environment.
• Observe and report changes to nursing or care team and document appropriately.
