Job summary

CHRISTUS Spohn Hospital Corpus Christi - Shoreline is seeking an Emergency Department Care Manager to optimize patient throughput and coordinate interdisciplinary care across the ED and post-ED settings.

The role involves collaborating with patients, families, physicians, social workers and payers to ensure appropriate levels of care and timely referrals, with focus on discharge planning and resource coordination beyond the hospital stay.

Qualifications

• Graduate of an accredited nursing program (BSN preferred) or Master’s in Social Work (MSW) required.
• 3+ years of relevant clinical case/care management experience in acute care setting.
• RN or LMSW license in the state of employment; Case Manager certification preferred; BLS preferred.

Responsibilities

• Coordinate patient assessment and admission criteria using approved criteria.
• Collaborate with physicians, nurses, social workers, and payers to determine appropriate level of care.
• Plan discharges including Home Health, DME, and community resources.
• Educate patients and families about care plans and potential barriers to care.
• Manage high-utilization patients and explore alternatives to frequent ED visits.
