Job summary

Qualifications

• Graduate of an accredited school of nursing (BSN preferred) or Master’s degree in Social Work (MSW) required
• RN or LMSW licensure in state of employment
• Strong knowledge of medical necessity criteria and prior authorization processes
• Proficiency in medical terminology and managed care language

Responsibilities

• Assess and coordinate patient care to determine appropriate setting and plan of care in the ED
• Review anticipated admissions and discharges using evidence-based criteria
• Educate ED staff and providers on medical necessity and admission criteria
• Coordinate post-ED referrals and community resources
• Facilitate discharge planning including Home Health, DME and community services
• Collaborate with interdisciplinary team to ensure timely patient progression and optimal outcomes
