Job summary

CHRISTUS Health is seeking an Emergency Department Care Manager to coordinate patient throughput and optimize care levels across the continuum. The role involves collaboration with patients, families, physicians, social workers, and payers to ensure appropriate levels of care and timely care planning.

The position requires nursing or social work credentials with relevant case management experience, including familiarity with medical necessity criteria and prior authorization processes.

Qualifications

• Graduate of an accredited school of nursing (BSN preferred) or Master’s degree in Social Work (MSW) required.
• 3+ years of relevant clinical case/care management experience in the acute care setting required.
• Competency in prior authorization functions and software, including the application of criteria and timelines required.

Responsibilities

• Establish, coordinate, and maintain the process to increase patient throughput to the most appropriate level of care.
• Collaborate with patients, families, and interdisciplinary teams to determine the appropriate level of care.
• Coordinate and facilitate patient progression throughout the continuum and discharge planning with referrals to all levels of care.
