Provident Behavioral Health is seeking an RN Care Manager to join the Emergency Department during night shifts. You will coordinate care for high-acuity patients and work closely with ED physicians, social workers, and community partners to ensure safe transitions.

You will perform comprehensive assessments, develop individualized care plans, and advocate for health equity, serving underserved populations in a fast-paced, mission-driven environment focused on teamwork and compassionate care.

Qualificações

• Active RN license required.
• Experience in emergency or acute care settings preferred.
• Strong assessment, care planning and coordination skills.
• Ability to work effectively with interdisciplinary teams.
• Commitment to serving underserved populations.
• Excellent communication and advocacy abilities.
