Job summary

Qualifications

Responsibilities

• Assesses the patient’s physical, psychosocial, and spiritual status.
• Performs ongoing assessments, reviews tests, and responds to interventions.
• Documents care per organizational standards.
• Coordinates discharge planning and referrals for continuity of care.
