Sollis Health is seeking an Advanced Care Provider (PA/NP) for at-home, concierge visits in the New York area. You will conduct in-home assessments, provide urgent/emergency-level care under physician supervision, and deliver high-touch care across adult and pediatric patients during overnight shifts.

The role supports a growing, membership-based model with a focus on reducing ER visits by bringing comprehensive care to patients’ homes.

Formación

• Licensed PA or NP with NY practice eligibility and DEA license.
• 2–3 years in emergency/urgent care preferred.
• Active NY State license and board-eligible certification.
• BLS/ACLS/PALS certification required on hire.
• House call, EMS/pre-hospital or concierge experience a plus.
• Excellent clinical care and patient satisfaction in ED/urgent care.

Responsabilidades

• Form differential diagnoses and treatment plans in home or non-clinic settings.
• Coordinate with House Call Coordinators and respond to requests promptly.
• Perform medical evaluation and use home imaging as part of assessment.
• Maintain stocked provider bag to avoid delays.
• Conduct Covid-related house calls with full PPE and testing.
• Perform IV access, fluid administration, splinting, suturing, phlebotomy and EKGs.
• Document encounters in EHR (eClinicalWorks).
• Communicate empathetically with patients and families.
• Interface with PCPs and specialists for follow-up care.
• Collaborate with Medical Director and clinical team to ensure policy adherence.
• Coordinate hand-offs of labs/imaging to in-clinic providers.
• Coordinate care with After Care and house calls teams; cover shifts as needed.
