Job summary

Sollis Health is seeking an Advanced Care Provider (PA or NP) for our At Home concierge team in New York. You will conduct in-home visits under the supervision of an attending physician, delivering urgent and routine care to adults and children, reducing ER visits by treating patients at home.

This full-time role supports 9a–9p shifts, collaborates with house-call staff, performs exams, IVs, POC testing, EKGs, and documents in eClinicalWorks.

Qualifications

• Bachelor’s degree completed; licensed PA or NP.
• New York State license to practice.
• 2–3 years in emergency or urgent care preferred.
• DEA license required; BLS/ACLS/PALS certification on hire.
• Excellent clinical care and patient satisfaction.

Responsibilities

• Conduct home-based evaluations and manage urgent/emergent care per scope.
• Coordinate with House Call Coordinators and respond to requests.
• Perform clinical assessments with point-of-care testing and EKGs.
• Document care in the electronic health record (eClinicalWorks).
• Interface with PCPs and specialists for follow-up care.
• Maintain stocked house call provider bag and supplies.
• Cover shifts at Sollis' flagship centers when needed.
