Job summary

Monarch Communities is seeking a Care Manager (CNA/CHHA) for the full-time night shift from 11:00pm to 7:00am at our Englishtown, NJ community. The role is non-exempt and hourly, with per-diem options available.

You will assist residents per their care plan, focusing on personal care, meals, and daily activities while maintaining safety and dignity. Responsibilities include documenting services, monitoring changes in residents, and collaborating with the Health & Wellness Director.

Qualifications

• Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) is preferred.
• Experience with elderly residents; dementia experience is a plus.

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding per care plan.
• Maintain safety, comfort, dignity, and confidentiality of residents.
• Escort residents to meals and activities.
• Ensure residents are clean, neatly dressed, safe, and fed during shifts.
• Document all services provided during the shift.
• Report changes in a resident's condition immediately to the supervisor.
