Job summary

Albany Med Health System is seeking a Patient Care Assistant to support the nursing team in the Cardio Pulmonary Surgery unit during night shifts. You will collect patient data, assist with personal care, and help with mobilization and discharge planning.

The role includes performing basic procedures such as blood pressure checks and glucose testing, maintaining a clean, safe environment, and documenting patient progress for the RN.

Qualifications

• High School Diploma/G.E.D. or equivalent required.
• Less than 1 year experience in a health care setting or completion of certified nurse assistant course preferred.
• Experience as a paramedic or emergency technician preferred.
• Basic knowledge of medical terminology (Medium proficiency).
• Ability to communicate cooperatively and effectively with patients, family, staff.

Responsibilities

• Collects data including vital signs, height/weight, and intake/output.
• Reports abnormal findings to the RN and care team.
• Participates in care planning and nursing report process.
• Provides personal care to patients including bathing, oral and skin care.
• Assists with eating, hydration, grooming, dressing and toileting.
• Documents all care in the patient record.
