Responsibilities

• Perform and record patient vital signs and intake/output.
• Observe patients' behavior and mental status; notify RN of changes.
• Assist with patient safety checks and de-escalation as directed.
• Escort patients for activities and tests; monitor access and movement.
• Document in EHR and Nursing Assessment forms.
• Participate in treatment team meetings and patient care planning.
• Provide personal care and comfort to psychiatric patients.
• Assist with one-to-one observation for high-risk patients.
• Support orientation of new staff and therapy groups.
