Qualifications

• Active IL nursing license required or currently licensed in the state.
• Clinical experience in skilled nursing or acute care preferred.

• Excellent communication, organization, and leadership abilities.

Responsibilities

• Provide individualized care to residents per care plan.
• Monitor residents and promptly report changes to licensed staff.
• Maintain accurate EMR documentation (e.g., Matrix-Care).
• Support safety, infection control, and regulatory compliance.
• Collaborate with interdisciplinary team to ensure smooth care transitions.
