Qualifications

Responsibilities

• Receives, interprets, and reviews prescriptions and patient medication history.
• Analyzes patient data to optimize drug therapy and provides clinical interventions.
• Oversees preparation and dispensing of medications, including sterile products.
• Ensures formulary compliance and cost-effective substitutions.
• Documents in medical records and other required systems.
• Counsels patients and families on pharmaceutical care issues.
• Directs shift workflow and supervises technicians, students, and volunteers.
• Complies with all relevant regulations and policies.
• Identifies process improvement opportunities and participates in training.
