Qualifications

• Skilled nursing care in home and school settings.
• Assess and monitor patient’s condition and communicate changes with the care team.
• Administer medications, treatments, and prescribed therapies.
• Provide trach, ventilator, airway, and G-tube care as needed.
• Maintain documentation and support families through education.
• Follow individualized care plans and maintain a safe care environment.
