Job summary

Bluestem Communities is seeking an experienced Overnight On-Call Nurse (LPN) to provide comprehensive home nursing care in the McPherson, KS area. The role involves wound care, IV hydration, lab draws, and collaboration with physicians and families to develop holistic care plans.

On-call shifts run Sunday through Thursday from 4:30 pm to 8:00 am, with duties including emergency response, documentation, and coordination with the Clinical Director for continuity of care.

Qualifications

• Must be a graduate of an accredited School of Nursing and licensed to practice in Kansas.
• Must have a valid Kansas driver’s license and current car insurance.
• Prefer at least 2 years of home care nursing experience, including with the frail elderly.

Responsibilities

• Administer wound care, IV hydration, lab draws and other nursing tasks during home visits.
• Collaborate with physicians, families and multidisciplinary teams to create holistic care plans.
• Respond promptly to provider communications and document interactions.
• Take immediate action in emergency situations and provide ongoing support for participants.
• Contribute to quality improvement through feedback and staff development.
• Communicate with Clinical Director before the end of shift to ensure morning meeting is prepared.
