Job summary

Mercy San Juan Medical Center, part of Dignity Health, seeks an on-call Hospice Registered Nurse (RN) for per diem home visits in California. The RN provides compassionate end‑of‑life care, coordinates with a multidisciplinary team, and supports families when the patient is home, including after-hours admissions when needed.

The role requires California RN licensure, BLS certification, and a valid driver’s license with access to a vehicle; prior hospice experience is preferred.

Qualifications

• Minimum one year hospice nursing experience in the last five years.
• Active RN license in California.
• BLS certification by American Heart Association.
• Current California driver's license, auto insurance and access to a vehicle.

Responsibilities

• Provide direct patient care in home hospice settings.
• Coordinate with physicians, nurses, social workers and other team members.
• Be on-call after hours for patient needs and potential admissions.
• Meet productivity standards and document according to policy.
• Communicate care plans across disciplines and record progress.
• Ensure compliance with state and federal regulatory requirements.
