Job summary

Night On Call Per Diem Hospice Registered Nurse (RN) at Dignity Health - Mercy San Juan Medical Center in Carmichael, CA, delivers compassionate home-based care to hospice patients and families, helping them manage a prognosis of six months or less. The on-call shift runs 1700 - 0830.

You will coordinate with physicians, nurses, social workers, chaplains, home health aides, and volunteers, while meeting productivity standards and ensuring compliance with agency, state, and federal regulations.

Qualifications

Minimum one year hospice nursing experience in the past five years.
Active RN license in California.
BLS certification from American Heart Association.

Responsibilities

Provide direct patient care during on-call shifts in patients' homes.
Coordinate care with physicians, nurses, social workers, chaplains, and home health aides.
Meet discipline productivity standards and document per policy.
Ensure compliance with state and federal regulations and agency processes.
Document progress and communicate care plans to the interdisciplinary team.
Respond to after-hours patient needs and assist with admissions when required.
