Job summary

Parker Jewish Institute for Health Care and Rehabilitation in New Hyde Park, NY, is seeking a Part-Time Nurse Practitioner for the 11:00pm–7:00am weekend shift to join our Medical Department.

You will examine residents, deliver quality care across subacute, long-term, rehab and outpatient settings, and support education for residents and families while updating care plans and documentation as needed.

Qualifications

• Two or more years of experience in a sub-acute or long term care facility.
• Graduate of an accredited nurse practitioner program.
• Active license as an NP in New York State, with prescriptive privileges.
• NYS Board Certified is required.
• Proficient in SigmaCare or another EMR software.
• Prior experience with the geriatric population is a plus.

Responsibilities

• Examine residents and deliver quality care in subacute, long-term, rehab and outpatient settings.
• Assist in resident/patient/family education and re‑examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts.
