Qualifications

Responsibilities

• Administer medications directly to residents as prescribed by physician and record deliveries on logs.
• Secure medications per policies and label bedside orders properly.
• Document medications administered on MARs and monitor PRN meds.
• Follow procedures for destruction of discontinued medications.
• Report changes in residents' condition promptly to leadership.
• Provide bathing assistance and daily living support.
• Assist with dressing, grooming, oral care, and hygiene.
• Assist with toileting, perineum care, and incontinence support.
• Assist with mobility and transfers per protocols.
• Monitor residents at risk and document observations via approved tools.
• Maintain accurate documentation and communication with the Director of Resident Services.
