Qualifications
- Current, active Licensed Vocational Nurse (LVN) license in good standing.
- Strong clinical judgment and communication skills.
- Home health or pediatric experience preferred but not required.
- New graduates encouraged to apply in select markets.
Responsibilities
- Deliver skilled nursing care in accordance with each patient's individualized plan of care
- Monitor patient conditions and communicate changes to the care team
- Administer medications, treatments, and therapies as ordered
- Educate and support patients and family caregivers
- Collaborate with physicians, therapists, supervisors, and care team members
- Maintain accurate and timely clinical documentation
- Advocate for the health, safety, and well-being of every patient
