Job summary

Regency Integrated Health Services in Edna, TX, is seeking an LVN Charge Nurse for a 6pm-6am shift in a skilled nursing facility. You will help ensure quality care and residents' well-being according to assessments and care plans.

The role requires completion of an LVN program and an active Texas license; prior SNF experience is preferred. You will collaborate with physicians, document visits, manage narcotics with shifts, and communicate with residents' families about updates.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.
• Must hold an active Licensed Vocational Nurse license in the state of Texas in good standing.
• Experience working in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition changes during rounds.
• Ensure care plans are implemented and medications administered as prescribed.
• Conduct rounds with physicians and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify significant changes and take action per facility policy.
• Oversee and monitor aides and discharge residents per policy.
• Complete Nursing Assessments prior to physician calls and communicate status updates.
